The Family Case Management (FCM) program provides comprehensive service coordination to improve the health, social, education, and developmental needs of pregnant women, and infants (0-12 months) from low-income families in the communities of Illinois.
Who can receive these services?
If you are pregnant, have a child under the age of one, and are on Medicaid, or part of a low-income family you may be eligible.
What services are offered?
The Family Case Management program can help you:
connect with the appropriate provider for your medical needs
understand the stages of your child's development
get information on how to become a better parent
understand the importance of prenatal care
learn and understand signs of normal and alarming pregnancy
understand the importance of regularly scheduled well child visits
understand the importance of immunizations
connect you to other programs that may be beneficial to you or your child
provide educational feedback on the importance of Safe Sleep practices
