Childhood immunizations are available through the Vaccines for Children (VFC) program for all children who do not have insurance, with insurance that does not cover immunizations, or children who are Medicaid eligible.
Immunization requirement: a meningococcal conjugate vaccination (MCV4) is now a requirement for students entering grades 6th or 12th. Sixth graders must show proof of one dose of MCV4; 12th-graders must show proof of having received two doses. Students in grade 12 who received the first dose after 16 years of age need only one dose.
Any child entering kindergarten, sixth, seventh, ninth, 10th, or 11th grade for the first time must also show proof of having received two doses of varicella (chickenpox) vaccine.

Vision & Hearing Screening
- All mandated screening must be done by screeners trained and certified by the Illinois Department of Public Health.
- Minimum mandated grades for vision screening are kindergarten, second, and eighth.
- Minimum mandated grades for hearing screening are kindergarten, first, second, and third.
- All children aged 3 and older in licensed daycare/preschool facilities must be screened annually for both vision and hearing.
School & Sports Physicals

Types of physicals available include:
- Preschool/Daycare/HeadStart
- Foster Care
- Kindergarten
- Sports Physicals for Grade School, High School, and College
- Freshman Physical for High School and College
Insurance and Medical Card will be billed. Reasonable accommodations will be made for uninsured children. Charges will be billed to private insurance or Medicaid when available. A basic fee will be charged at the time of service to those without insurance.
